It's lichen. Grows on exposed lace rock above ground. It's not growing now, it's being eaten by other bacteria/algae/fungus. I would power wash it off. Or take a steel-bristle brush to it.

Many internal infections are too far advanced to treat once the fish are showing external symptoms.

The impeller probably was dislodged. When you cleaned it you got it back where it belonged. It's possible that the small bushings at the ends of the axle need to be replaced.
