Does this angelfish have hole in the head?

It definitely isn't ich....only other thing I can think of is hole in the head. I recently lost a lot of lemon tetras from something that would eat away part of their face gills and body, like lesions. Could it have been hole in the head all along? here are some videos of the tetras and another of the angelfish. If not what in the world is it and what should I use? The lemon tetras are replaceable but the angel is the star of the show.

I'd be very interested in hearing what other people think. I saw this exact thing on one of my angelfish once as well (nearly the same location, too). I strongly suspect that it's a secondary fungal infection, but it's always possible it was bacterial. I did my usual treatment....a dose of both Melafix and Pimafix. it went away quickly, but left a small crater behind.

I don't know it isn't my tank and I never tested it initially. I just go every other week to do maintenance on it. Fish had problems when i first started taking care of the tank I removed them all put in a uv sterilizer and got all new fish. They were fine for months then the lemon tetras started getting this again, now the angel has it. The tank should be fine it gets at least 50% water change every other week.
